GENERAL PURPOSE:The Clinical Coordinator assigned to the Comprehensive Psychiatric Emergency Program (CPEP) has primary responsibility and accountability for the CPEP workflow on their shift. The Clinical Coordinator provides leadership, direction and support to the team in moving patients through the evaluation process. The Clinical Coordinator monitors productivity of CPEP Clinical Evaluators on assigned shift, while serving as direct supervisor, clinical resource person, Patient Advocate, and clinical role model. As a member of the interdisciplinary leadership team for the CPEP, the Clinical Coordinator collaborates to develop and implement policies, procedures, and performance improvement plans; and ensures that policies and procedures are followed with regard to patient care, discharge planning, and professional practice. CPEP is a 24/7 psychiatric emergency program serving all ages who present with acute behavioral health symptoms. The Clinical Coordinator works shifts as assigned by the CPEP Program Director.JOB DUTIES AND RESPONSIBILITIES:Provides management and direction for the CPEP Clinical workflow on their shift.

This includes directing the order in which patients are seen for evaluation/re-evaluation, prioritizing and re-prioritizing as needed; Problem- solving barriers to patient care, assigning tasks to CPEP Clinical Staff as needed.

Provides support and intervention during times of high volumes or extensive wait times .

This includes evaluating need and approving and/or directing requests for additional staff help; Informing and directing CPEP

Team as needed about responding to patient and family needs and requests.

Monitors productivity of CPEP Clinical Evaluators on assigned shift by :

Providing individual CPEP Clinical Evaluators direction as needed;

Notifying appropriate supervisor of any needed follow-up; Clarifying policy or preferred practice as needed; Problem-solving with on-shift CPEP Team as needed to address problems impacting patient flow.

Serves as a Clinical Resource person, Patient Advocate, and Clinical Role Model by :

Provides information and instruction as needed by CPEP Clinical Staff to consistently meet standards of care; advocating for all patients; provides direction, instruction or resources needed for CPEP Team to meet patient needs or address patient/family concerns and complaints (service recovery) in the moment.

Develops and maintains linkages with community agencies/services integral to continuity of care within the service. Provides a Role Model for consistent professional, compassionate and calm interactions with patients, families, and other providers/professionals.

Coordinates complex discharge planning with community providers and family/guardian, advocating for patient needs related to safe discharge; keeps CPEP Leadership Team and Department/Hospital administration appropriately apprised.

Serves as a member of the CPEP interdisciplinary Leadership Team:

Represents CPEP in meetings with and education to community service providers; Collaborates to develop and implement policies, procedures, and performance improvement plans; Ensures that policies and procedures are implemented with regard to patient care, discharge planning, and professional practice; Represents CPEP Leadership Team in all interactions with staff, patients and families, supporting identified goals and expectations.

Participates in activities of the Psychiatry Division including, but not limited to, timely completion of time reporting and other Hospital or Division forms; Hospital and Division mandatory in-service training programs and UHS

reviews. Documents all interventions and contacts in patient's chart in compliance with Division, Department and Hospital policy.

Subscribes to the Departmental goal of creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences.

Other duties as assignedQUALIFICATIONS:Required:

Master¿s degree from a Counsel of Accreditation of Counseling and Related Education Programs (CACREP) accredited program with major course work in mental health counseling.

Prior experience working in acute psychiatric care setting.

Two years of clinical experience post-graduation from Masters¿ Program.

Clinical knowledge and ability to provide behavioral health assessments/evaluations, crisis intervention, safety planning and referrals for community-based services and supports.

Ability to direct and redirect clinical staff in a collaborative manner

Commitment to and routine demonstration of biopsychosocial approach to care of individuals and families

Dedication to delivering a team-based integrated model of care

Demonstrates cultural humility and anti-racism values and works toward ensuring all patients feel welcome.

Requires :

NYS licensure as a behavioral health practitioner (e.g. LCSW, LMSW, Mental Health Counselor, CASAC)

or limited permit eligible.

If licensure has not been obtained upon employment, the individual has 18 months to obtain the NYS license and must obtain limited permit upon hire.
