As a Manager of Care Coordination, you will discuss discharge needs with Physicians and Social Workers; and partner with unit nurses to communicate with patients/family about admission and plan of care. Job Details Requirements include: Active NY Registered Nurse license 2-3 years experience in health care related discipline and/or discharge planning or community healthcare experience BSN preferred Excellent interpersonal skills Ability to respond to short deadlines Salary Range USD $49.65 - USD $68.10 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits. DUTIES/RESPONSIBILITIES: Develop and implement a risk management program for the physician practices, which includes, but is not limited to, risk identification and harm and loss prevention/ and reduction. Conducts RCA’s for serious safety events and manages the high reliability safety program in coordination with ambulatory quality. Responsible for all aspects of incident investigation. Reports all potential| claims to the insurance carrier. Develops an educational program and an orientation program for staff on risk management and safety topics. Assists with discovery and management of litigation matters. Provides guidance for compliance with regulatory inquiries, complaints and clinical care Incidents. POSITION REQUIREMENTS AND QUALIFICATIONS: Education: BSN, Masters, CPHRM Skills: Excellent communicator, proficient in Microsoft Office Physical Requirements: Travel across LI to medical practice sites Experience: 3 plus years in Risk management Salary Range USD $117,000.00 - USD $149,000.00 /Yr.
