OverviewThe Community Residence Specialist supports the daily activities in a community home serving older adults with psychiatric disabilities with a goal of getting them to the least restrictive level of care. The Specialist promotes a safe and respectful home environment while providing guidance and support to all members in the home. With the Community Residence team, develops and implements individual service plans with specific goals, tasks and methods of measuring progress. Works with residents in all activities, including daily living skills, meal preparation, interpersonal and communication skills, medication management, crisis resolution, medical visits, transportation and activities promoting independence. Maintains contact with programs and service providers who work with the residents. This is a full time position with rotating on call responsibilities covering three sites.ResponsibilitiesJob duties specific to this position:

Handles case management responsibilities for an assigned group of residents.

Completes service plans and utilization reviews for each resident and updates as needed. Involves resident, family and counselor in goal setting and progress assessments.

Responsible for medication management. Trains staff in medication program. Reviews medication book daily for accuracy and prescription refills. Orders medication refills and/or new scripts as needed. Prepares monthly medication sheets. Monitors staff performance for accuracy as per medication program.

Serves as a liaison between physicians and residents concerning medication changes, possible side or adverse effects and purpose of medications. Updates residents’ medication sheets as needed.

Develops a monthly menu with the residents and counselors. Prepares shopping lists and purchases food, using food stamps, the food bank, coupons and other cost effective measures to stay within the food budget. Prepares grocery receipts for reimbursement.

Pursues food stamps for eligible residents and completes re-certifications as needed.

Consults with Loretto Nutritionist or Food Bank Nutritionist to provide training for staff and residents as needed.

Provides resident advocacy and linkages to ensure continuity with day programs, legal, medical, dental, psychiatric and social services.

Completes or updates other program/resident documentation, including but not limited to progress notes, URs, discharge paperwork, etc.

Coordinates resident activity programs with a goal of enriching or improving resident lives. Applies input from residents and counselors. Evaluates activity outcomes.

Serves as liaison between providers and residents to ensure Medicaid reinstatement of services for transportation.

Transports residents to and from day programs and/or appointments as needed. Schedules repairs, servicing and maintenance of van. Oversees mileage logbook completion.

Provides support and assistance in crisis/problematic situations as needed. Handles crisis and non-crisis situations using sound judgement and input from staff, providers, residents and significant others.

Delegates tasks to counselors and follows up to ensure completion.

Keeps Site Supervisor updated on resident progress, resident issues and staff performance at all times. Meets with Site Supervisor on an ongoing basis to discuss administrative and direct care goals and accomplishments.

Collaborates with other service providers in a professional manner.

Develops an appropriate rapport with residents. Demonstrates sensitivity and responsiveness to residents’ needs when counseling.

Maintains resident confidentiality and shares information appropriately with team members.

Acts as a positive and effective role model to staff and residents at all times. Demonstrates a sensitive and responsive approach with coworkers.

Provides supervision of the community residence when Site Supervisor is not available.

Assists with interviewing and hiring counselors and on-call counselors.

Is flexible and able to work evenings and cover weekends as needed or scheduled.

May assume additional duties to meet the needs of the residents or support the effective operation of the Community Residences.

Qualifications

Bachelor's Degree in a human service field, or Associate's Degree in a human service field and at least three years experience in human services, or a High School Diploma and 5 years experience in human services.

A valid and clean New York State Driver’s License.

Willingness to be flexible and perform extra duties as needed.

Ability to work effectively within a team and also independently.

Ability to multitask and meet deadlines.

Ability to work rotating shifts in a 24/7 environment and take call as needed.

Proficient with MS Office and able to learn new software rapidly.
