50.00-100.00 per visit | NY, USA | Other | Clinic | Contracted PHYSICAL THERAPIST - FEE FOR SERVICE CONTRACT Mozaic is looking for a Fee-for-Service Physical Therapist to provide physical therapy services to improve or limit a disabling condition through the provision of professional therapies. Services are provided to persons who require such services to remain in or move to the least restrictive environment or because such services are unavailable or inaccessible in the person's community. PAY RANGE: $50-$100/hour - Fee for service contract LOCATION: Seneca, Cayuga or Yates County PHYSICAL THERAPIST DUTIES Provides treatment and training to help individuals maintain independence by improving motor skills, strength, coordination, and daily activities while preventing further disabilities and supporting heart and lung health through therapy and adaptive equipment. Provides direct physical therapy services by specific written medical prescriptions. Enhance the effectiveness of the treatment, enable the person to remain in the family or placement setting as long as desired, and enhance the person's quality of life. MINIMUM QUALIFICATIONS License and current registration as a Physical Therapist by the NYS Education Department. Experience with developmentally disabled preferred. Experience with working with pediatrics is required.
