Retention and Adherence Program Specialist - Full Time - Days Department: MG - SPEC - INFECTIOUS DISEASE Job SummaryThe RAP Specialist (Retention and Adherence Program) is responsible for managing a caseload of NYS RAP Grant program patients engaged with the MVHS Immune Health Clinic. Coordinates ongoing patient care with behavioral health providers, case manager providers, other medical providers, primary care providers, social services provider, whether internal or external, both on-site and off-site to ensure regular engagement with HIV medical care, promote adherence, and address medical, psychosocial, socio-emotional, and non-medical social determinants of health (SDOH) needs of patient.Core Job Responsibilities

Performs assessment and evaluation of all aspects of barriers of care for each patient on program in the outpatient setting of HIV/AIDS positive patients.

Responsible for comprehensive assessment that will be inclusive of medical/psycho-social issues including medical care, retention/compliance, treatment adherence and prevention tailored to each patient need.

Acts as an advocate for patients and families to ensure that they are aware of resources available to them in the community.

Collaborates with providers, nursing staff and other grant staff to ensure that patients are receiving the appropriate level of care and needs are being met.

Provides skilled knowledge and tools to employ strategies that will ensure compliance, while coordinating care and access to range of appropriate medical and social services.

Employs specific training techniques and theories, including EBI’s (Evidence Based Intervention) and other high impact prevention strategies to assist in the creation of individualized care plans and evaluations, including assessments for behavioral health and substance use treatment readiness.

Uses training strategies to promote proactive involvement in care, advocates for patients and provides them with appropriate linkages/information regarding available community resources.

May provide on-site and off-site care relating to patients. This may include traveling to any appointment related to HIV Medical care and any Social Determinants of Health (SDOH) care, such as going to the county building, bringing the patient to appointments, meeting them at their home or in the community.

Ensures there is clear documentation of services offered, received and anticipated to ensure all patient needs are met.

Perform other duties as required and outlined through the AIDS Institute.

Performs related duties as assigned.

Education/Experience RequirementsREQUIRED:

Bachelor’s degree in related field and one (1) year of HIV or other chronic illness related field experience OR Associate degree and three (3) years of HIV/chronic illness related experience.

Active and valid driver’s license.

Access to a reliable vehicle with insurance for appointments within the community.

PREFERRED:

2-3 years’ experience working with HIV/AIDS population, working with grant programs.

Active and valid driver’s license.
