Day Shift (Monday through Friday, 8:00am to 5:00pm with occasional evening hours to meet patient needs.)POSITION SUMMARY:This nurse care coordinator position will work with the STEPS program(Pregnant and Postpartum women with substance use disorder). Assesses, plans and implements all aspects of the Healthy Beginnings Plus/STEPS program to promote and facilitate healthy prenatal care. Orchestrates total patient care by scheduling and attending appointments with HBP patients and interacting with attending and resident staff and other healthcare providers involved in Healthy Beginnings Plus/ STEPS care. Patient care includes care to patients in the following age groups: neonatal (birth to 30 days), pediatric (30 days through 12 years), adolescent (13 through 18 years), adult (19 through 64 years).The Healthy Beginnings Plus Care Coordinator Colleague possesses the knowledge, skills and the attitude to effectively meet standards for competent professional nursing practice. The Healthy Beginnings Plus Care Coordinator Colleague exhibits Penn Medicine's experience standards which reflect its mission and are embedded in the professional practice competency domains. The Healthy Beginnings Plus Care Coordinator Colleague independently provides patient care through the application of the nursing process and accepts accountability for the nursing care of assigned patientsThe Healthy Beginnings Plus Care Coordinator Colleague demonstrates within the competency domains of Continuous Quality Improvement, Evidence Based Practice and Research, Leadership, Person and Family Centered Care, Professionalism, Safety, Technology/Informatics, and Teamwork.The Healthy Beginnings Plus Care Coordinator Colleague works closely with the multi-disciplinary health care team to facilitate the coordination of care across the continuum. The nurse focuses on expanding the knowledge and skills necessary to provide individualized care based on physical, psychosocial, cultural, educational, safety, and age-appropriate considerations of assigned patients.The Healthy Beginnings Plus Care Coordinator Colleague seeks feedback in assuming leadership roles and provides feedback for improved clinical practice. The nurse demonstrates how nursing practice impacts the organizational vision, mission, and goals and the care delivery system. The nurse complies with all regulations and standards of regulatory and accrediting bodies.ESSENTIAL FUNCTIONS: Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:

Performs patient assessments, focusing on physiological, cognitive and social functions.

Formulates a plan of care based on the patient assessment program guidelines and patient outcomes.

Provides educational materials and instruction to patients regarding health and prenatal wellness issues (smoking cessation, diet therapy, etc.). Education provided through home visits or in the office or community setting.

Provides counseling and social support to patients. Communicates with both healthcare providers and non-healthcare agencies/persons to obtain resources necessary to meet the needs of patients.

Evaluates patient’s living conditions through home visits and takes necessary steps to establish healthy environment.

Documents relevant patient care information and maintains detailed statistics regarding HBP patients.

Completes or supervises the completion of all paperwork associated with the HBP program to ensure proper reimbursement is received.

Evaluates effectiveness of HBP program and progress toward patient outcomes.

SECONDARY FUNCTIONS: The following duties are considered secondary to the primary duties listed above:

Monitors and updates HBP patient education library specific to patient needs. Develops and implements new educational media and programs.

Other duties as assigned.

JOB REQUIREMENTSMINIMUM REQUIRED QUALIFICATIONS:

Interest in working with those affected by Substance Use Disorder

Willing to do education and training related to Substance Use Disorder and Medication for Opiate Use Disorder to achieve/enhance the skills and knowledge for case management

High School diploma/GED.

Current licensure as a Registered Professional Nurse, issued by the PA State Board of Nursing.

CPR certification through the American Heart Association.

Two (2) years of previous nursing experience in Maternal/Child Health or other related health field with direct patient care.

Current PA Driver's License.

PREFERRED QUALIFICATIONS:

One (1) year of newborn nursery/OB and/or one (1) year of child development experience

Two (2) years of ambulatory care

Bachelor of Science degree in Nursing (BSN)

Two (2) years of social services agency interaction.

Sound experience with or knowledge of public health systems.

Experience in working with individuals with substance use disorder
