Hours of Work :8a-5pDays Of Week :Mon - FriWork Shift :8X5 Day (United States of America)Job Description :Your Job:Job Purpose: The Clinical Educator is an RN and will work with a multi-disciplinary team at the Golden Cross Academic Clinic to promote appropriate care for the ambulatory population with chronic medical conditions, including diabetes, hypertension, congestive heart failure, hyperlipidemia, asthma and other conditions by providing both clinical care and education to the patients and their families. Bachelor degree in Nursing preferred Graduate from an accredited school of nursing. Certified Diabetic Educator, or eligible to become a Certified Diabetes Educator Current CPR Licensed in Texas as a Registered NurseYour Job Responsibilities : Contributes to and participates in continuous quality improvement process to measure improvement and opportunities for improvement. Appraises performance to identify areas of strength and areas for improvement and to develop a plan for improvement and growth. Creates and oversees implementation of chronic disease self management tools and resources for patients. Works closely with a multi-disciplinary team, including physicians, nurses, pharmacist, and social worker to develop new and update current policies and procedures for providing care. Designs and conducts community educational programs for patients, various public groups, and health care professionals based on analysis of patient and community needs. Develops and promotes educational/informational materials and workshops that are accurate, culturally appropriate and educationally sound. Provide clinical education for outpatient population. Assess and identify the patients' individual needs and barriers to providing education. Collaboratively develop educational goals, learning objectives and a plan for educational content and teaching methods. Evaluates effectiveness of educational services provided by measuring attainment of learning objectives. Documents assessment data, educational plan, educational services provided and evaluation results in each patient's medical record. Utilize a team approach to providing services and collaborates and communicates with team members. Provide nursing support for patients in the MedAssist program.
