Your ResponsibilitiesAs a Hospice RN Case Manager , you will:
Provide patient care across the continuum, managing hospice services with empathy and expertise.
Assess, plan, implement, and evaluate individualized care plans for patients.
Support patients and families through end-of-life care with compassion and dignity.
Collaborate with interdisciplinary teams to ensure seamless care delivery.
Maintain accurate documentation and manage multiple priorities effectively.
What You Bring
Education : Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate Nursing Program.
Licensure : Current Registered Nurse (RN) license in [Insert State].
Experience :
Minimum of 1 year in a clinical care setting.
Hospice or home health experience (preferred).
Skills : Strong nursing skills, ability to manage priorities, and dedication to patient-centered care.
Transportation : Reliable transportation to perform job responsibilities.
