Position SummarySovah Health is looking for the ideal candidate who willbe the first line of contact for a patient in the medical office. Position serves as the liaison between the physician and the patients. Information is gathered from the patient and entered into the computer system to create an encounter record. Responsible for upfront collections. Delivers customer service support to staff, patients, families, visitors.Greets patients and visitors.Registers patients, verifies patient information, explains and obtains necessary signatures.Answers, screens, and routes telephone calls on a multi-line phone system.Communicates with the answering service and ensures reminder phone calls are done daily.Collects patient co-payments, deductibles, payments and assists patients with account balances and questions.Reconciles cash/payment drawer and for the daily bank deposit.Follows through on MD orders by providing work excuses, obtaining medical records.Assembles and maintains patient records and charts.Who We Are:People are our passion and purpose.Come work where you are appreciated for who you are not just what you can do.   • Job Summary The purpose of the Ambulatory Care Coordinator/Transition Manager is to deliberately organize patient care activities between two or more participants (including the patient) involved in a patient’s care to facilitate the appropriate delivery of health care services, ensure that the patient’s needs and preferences for health services are met and information sharing across people, functions and sites are met over time. Care Coordination maximizes the value of services delivered to a patient to by facilitating beneficial efficient, safe and high quality patient experiences and improved health care outcomes. Transitional care provides a broad range of services deigned to ensure health care continuity, avoid preventable poor outcomes among at-risk, complex and chronic condition populations, and promote the safe and timely transfer of patients from one level of care to another or from one type of setting to another by using evidence based care planning, comprehensive assessment, coordination of services and critical thinking skills.
