Job SummaryCoordinates and supervises the delivery of nursing care and activities for the Hospital at Home program. Screens, evaluates, and coordinates enrollees, and provides direct supervision and patient care. Assists in developing, initiating, and supporting unit-level improvements to deliver high-quality, safe, and cost-effective patient care. Ensures the highest level of care, optimizing all customer interactions to maximize patient satisfaction.Job Duties
Supervises assigned personnel.
May provide direct patient care as needed
Assists with orientation and integration of new employees.
Screens potential new enrollees and evaluates needs for appropriate care programs.
Develops support materials regarding program goals and objectives.
Communicates program details to physicians and healthcare providers.
Identifies patients for recruitment and conducts outreach, including in-home visits.
Ensures appropriate segmentation and handoff to care teams.
Manages the delivery of acute care in the home setting, ensuring comprehensive and continuous patient support.
Assesses home environments for suitability and coordinates necessary modifications for safe and effective care.
Understands the scope and abilities of a variety of team members to ensure efficient and quality-based processes
Participates in the formulation and communication of nursing policies.
Conducts performance appraisals, schedules staffing, and handles patient triage.
Participates in IDT meetings and ensures regulatory compliance.
Handles acute care calls and coordinates response to care needs.
Documents enrollment processes and supports reporting requirements.
Prevents and resolves patient and family concerns.
Supports Magnet philosophies such as professional growth and patient involvement.
Mentors staff and promotes evidence-based practices.
Ensures integration of technology and telehealth services to enhance patient monitoring and care delivery in home settings.
