Applications will be reviewed as they are received, and the position will remain open until filled.Starting salary for this position is $19.75-$22.11 per hour DOEFull range is $19.75-$31.13 per hourBenefits include medical, dental, life, LTD, PTO and 403(b)Start your journey toward making an impact in our community!Position Summary: This Community Health Worker II position will be focused on providing culturally appropriate health education and information, conducting outreach, building relationships with referral sources, coordinating care and helping clients navigate the health and social determinants of health systems.Principle Duties and Responsibilities:Responsible for establishing trusting relationships with clients and their families while providing general support and encouragement.Responsible for safely transporting clients to and from appointments in a timely manner in SNAP owned vehicles, (transporting clients in personal vehicles is strictly prohibited).Perform pre-trip vehicle safety inspections.Assist clients to and from door, as well as, in and out of vehicle, as needed.Operate wheelchair ramp and securement devices appropriately, as needed.Accurately record mileage.Immediately report to supervisor any problems or concerns related to vehicle.Immediately report to supervisor any problems encountered or concerns observed while transporting clients.Maintain vehicle cleanliness with daily cleaning of the interior of the vehicle and weekly cleaning of the exterior of the vehicle.Gas vehicles up as needed to keep a minimum of ΒΌ tank.Manage assigned caseload of clients.Work with clients to assess and prioritize needs, formulate plans to address needs, implement plans and evaluate plans.Follow-up with clients from initial identification through closure, via phone calls, home visits and visits to other sites where clients can be found.Record client care management information in the agreed upon client database (training provided) and other software no later than 24 hours after client contact.Work closely with other community agencies, medical providers, behavioral health providers etc. to help ensure that clients have comprehensive and coordinated care.Be knowledgeable about community resources appropriate to needs of clients/families.Provide referrals for services to community agencies as appropriate.Act as a client advocate and liaison between the client/family and other agencies/ service providers.Assist clients with completing applications for services. (i.e.: food benefits, SSI, TANF, GAU, housing applications, etc.)Attend regular staff meetings and other meetings as requested.Attend quarterly trainings related to health specific topics.Work with coworkers and volunteers to promote a positive, respectful and productive working environment, while protecting client confidentiality and dignity.Within the context of the stated principal job duties, serve as an ambassad
