Description SUMMARY As a Certified Home Health Aide (HHA), you will provide care to clients in their home, following their plan of care. You make a positive difference each and every day by demonstrating care and compassion to our patients.STATUS: Full Time LOCATION: Cayuga CountyDEPARTMENT: Home CareSCHEDULE: Day/Evening ATTRIBUTESCompassionate, patient focused and dependable professionalCritical thinking skills, decisive judgment and the ability to work with minimal supervisionExceptional documentation and communication skillsAbility to read, write and speak English, ability to follow verbal and written instructionsRESPONSIBILITIES Patient Care & Service. Promote and restore patients' health by completing all activities listed in the care plan; collaborate with Community Health Nurse/Therapist; perform various activities such as personal care, light housekeeping, meal preparation and errandsCommunication. Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularlyDocumentation. Accurately and completely document client care on activity sheetCompliance. Adhere to required department and system protocols, HIPPA regulations, local, state, federal requirementsPHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.LICENSES/CERTIFICATIONS: NYS DL - valid New York State Driver's License - New York State Department of Motor Vehicles (NYSDMV)HHA - Home Health Aide certification - New York State Education Department (NYSED)PAY RANGE: $17.60 - $19.50The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future.
