ABOUT THE ROLEThe ICMS Case Manager provides participant-centered intensive case management, builds ongoing relationships with participants, and coordinates services across Care Campus and community providers to support housing stability, health, wellness, and continuity of care.WHAT YOU’LL DOComplete participant intake, assessment, enrollment, individualized service planning, and ongoing case-management follow-up.Connect participants to housing resources, healthcare, behavioral health, substance-use services, benefits, legal resources, and other supports.Complete CES and other required assessments, referrals, documentation, reassessments, and service coordination in designated systems.Work closely with Client Support Specialists and multidisciplinary partners while maintaining responsibility for formal case-management decisions and documentation.
