Bachelor’s Degree in Human Services or related area (MA Preferred).
5 years of direct hands-on experience working with individuals with a variety
of disabilities.
3-5 Years of Supervisory and management experience.
Ability to communicate effectively both orally and in writing. Professional
English writing ability, including usage, grammar,
spelling, punctuation, and vocabulary.
Ability to keep and file records.
Willingness to accept work direction from others.
Ability to conduct staff meetings and trainings.
Proficient use of basic office equipment, Microsoft Office.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Hire, train and evaluate the Transition Coordinator and Assistants. Recruit,
review and process referrals to the College Education Services
program.
Attend intake team meetings for new consumers.
Develop, implement, modify and review Individualized Support plans for
each consumer.
Based on assessment(s) and person-centered planning write individualized
goals and objectives for each individual.
Negotiate class schedule with prospective College when needed.
Per agency procedures, complete all required documentation and reports.
Complete billing information for Regional Center funding.

Obtain and ensure authorizations from funding sources.
Coordinate and conduct staff meetings.
Provide technical assistance at c o lle ge sites as needed.
Prepare and operate within annual budget in conjunction with the CEO
and CFO.
Attend all required meetings and in-person services.
Create and review weekly schedules for direct reports.
Provide support and motivation to co-workers, direct reports and
consumers.
Discipline of direct reports, up to and including termination, as needed.
Collect and audit time cards and mileage for direct reports.
Communicate about and submit employee paperwork regarding
Workers compensation, leaves of absences and employee updates as
required.
PERFORMANCE STANDARDS
Works in partnership with RD to maintain staffing levels.
Thorough and timely completion of performance evaluations.
Completion of training and documentation of all newly-hired direct
reports.
Monthly staff meetings and safety drills are held and documented.
Effective mentoring and development of direct reports in their job duties.
Timely completion of weekly schedules which provide a high
quality of experience for consumers.
Responds in a timely manner to the issues of co-workers, direct
reports, participants, families, outside agencies, and appropriate
others.
Participant files are complete, accurate and up-to-date.
ISPs are all current; goals and objectives are set, and progress is being
made toward the attainment of those goals.
Meets or exceeds budget goals.
Full and timely completion and forwarding of special incident reports.
Complies with all safety standards and practices.
Attends all meetings and in-services.
Timely, accurate and complete time cards and mileage reports.
Completes billing effectiveness monthly.
