What You Need to Know Clinical Leadership and Compliance: Supervises day-to-day clinical and office operations. Is available to staff and patients during hours of operation. Ensures services are in compliance with professional standards, agency policy and procedure, and state and federal regulatory requirements. Serves as back-up support for positions as needed: PCM, CSS. Leader in absence of the Territory - Administrator. Clinical Oversight of Patient Care: Coordinates referrals. Coordinates patient care with an interdisciplinary team approach. Assures the development, implementation, and updating of the individualized plan of care. Evaluates the quality of clinical care, ensuring that patient needs are continually assessed. Collaborates with clinicians and physicians regarding patient transfers or discharges. Reviews Discharge assessment data, Edit/Lock Discharge OASIS, and discipline only discharge. Daily management of KPIs, workflow, escalation/complaint calls. Guides professional clinicians on implementing Emergency Care Protocols. Clinical Oversight of Personnel: Assures clinicians including contract staff have required training, competency, and supervision. Makes patient and personnel assignments assuring that assigned staff have demonstrated competence in the required tasks/services. Ongoing coaching and mentoring of field clinicians. Quality and Compliance: QAPI owner and participation in QA and performance improvement activities. Reviews/investigates incidents, analyzes for trends; implements corrective action plans as needed. Accreditation/Survey Readiness, QA audits/detailed corrective action plan audits. Manages HHCAHPS distribution in SHP. Emergency Preparedness Planning- Activation of EPP, Post Activation Reporting & Ongoing Event Monitoring and Reporting. Qualifications Registered nurse with current licensure to practice nursing in the practicing state Possess and maintain CPR certification Previous experience in home care setting with two years management or supervisory experience, preferred Proven ability to work within an interdisciplinary setting Must be a licensed driver with an automobile that is insured and in accordance with state and/or organization requirements and is in good working order Prior use of Electronic Medical Records (EMR) preferred

  • Summary The primary purpose of the position is to provide direct service in the form of counseling individuals, couples, families and groups on matters pertaining to family dynamics, parent-child interaction, couples communication, stress reduction, adjustment to military lifestyles, Family Advocacy Program (FAP) case management, treatment services and psycho-educational training programs. Responsibilities This position is that of a fully licensed and credentialed clinical counselor located within the Counseling and Advocacy Program (CAP) at the Fleet and Family Support Center (FFSC). FFSC provides an array of quality of life programs to active duty, reservists, retirees, and their families to enhance personal and family resiliency. Family Advocacy Program (FAP) case management includes the identification, reporting, safety planning, psychosocial and risk assessments, treatment planning, treatment, referral, and follow-up of intimate partner violence and child abuse allegations for active duty military members and their families. Cases may involve multiple difficult and complex family problems including dual diagnoses, violence and impulsivity, suicidal and homicidal ideations and/or attempts, and a variety of other mental health problems. The incumbent also provides assessment, safety planning, case management, treatment, and referral for Problematic Sexual Behavior in Children and Youth (PSB-CY). The incumbent facilitates support groups, offender treatment, and seminars/training focused on both prevention and intervention. The incumbent occasionally provides individual, couples, family, and group counseling to active duty personnel and their families. Clinical counseling typically addresses life stressors, family dynamics, and adjustment to military life. Clinical counseling includes the psychosocial assessment of individuals, development of treatment plans, treatment delivery, and appropriate referral. The incumbent also occasionally provides Sailor Assistance and Intercept for Life (SAIL) case management, including: assessment, safety planning, and care coordination for active duty personnel with suicide related behavior. Plans, develops and implements programs designed to foster positive healthy relationships within the family structure. Requirements Conditions of Employment Qualifications A Master\'s degree or doctorate in Social Work, Marriage and Family Therapy, Counseling or Clinical Psychology or related field from an accredited institution from any US State. Have and maintain a current and valid state clinical license that permits the incumbent to function as an independent health practitioner and fully meet and maintain the Tier II or Tier III SECNAVINST 1754.8 requirements. Are current in continuing education credits/units to maintain license. Knowledge of the specific demands of the military lifestyle, such as frequent separations, and high stress working environments that have an extensive impact on individuals and their families preferred. Ability to present sensitive, at times controversial, information to groups of people in a clear, concise manner. Knowledge of dynamics and diagnostic skills necessary for family violence identification, risk assessment, and safety planning. Ability to effectively interview, and clinically respond to the needs of clients including assessing suicide, homicide, and substance abuse. Ability to communicate in writing to develop and prepare case management documentation, safety plans, psycho-social assessments, treatment plans, correspondence, and reports. Knowledge and ability to use a personal computer and a variety of software programs to develop multi-media presentations and to facilitate work assignments. Must be able to communicate clearly and effectively both verbally and in writing in English with all levels of management, staff, community agencies, military personnel, and their families.
