We are locally owned with over 200 years of combined professional experience in the industry and are adding an experienced Care Manager to our team. In this role you will get to make a meaningful and positive impact to the lives of our clients, caregivers, and organization and enable the compassionate and empathetic delivery of essential home care services.
Responsibilities:
Leadership:
Foster a culture of collaboration, empathy, and continuous improvement
Ensure caregiver happiness by supporting their professional needs and development, being an advocate for their success, and creating a culture of accountability
Client Acquisition, Growth, and Retention:
Own the client acquisition process, moving prospects through each stage of the funnel from initial inquiry to intake, assessment and start of care
Guide prospective clients and their families through a personalized consultation and care needs assessment
Assist with client retention strategies, ensuring client happiness and satisfaction
Monitor metrics and suggest new strategies to improve retention and growth
Regularly assess and “rightsize” client services to ensure appropriate levels of care
Build and maintain relationships with clients and their families as a trusted advisor
Care Delivery:
Ensure the development and execution of personalized care plans for each client
Oversee client assessments, contract negotiations, and service adjustments to match evolving needs
Ensure compliance with company’s standard of care
Lead quality assurance initiatives to maintain excellent service delivery
Resolve client concerns and escalations promptly and effectively
Collaborate with other departments to ensure seamless care delivery across all touch-points
Skills and Qualifications
RN is required
3-5 years of experience in home care, home health or other related field
Ability to effectively communicate with clients, families, and caregivers, often in a home environment
Strong focus on personalized care plans, client advocacy, and caregiver support
Empathetic and compassionate towards clients and caregivers
Ability to manage the logistics of coordinating care across different home settings and ensure high standards of service
Flexibility to respond to the unique and sometimes unpredictable nature of home care
Strong problem-solving skills to address and resolve issues that arise in a home care context
Ability to analyze data, identify trends, and use this information to improve care management practices
