Job Title/Position:  Registered Nurse/Clinical Manager

Reports To: Clinical Director

JOB DESCRIPTION SUMMARY

The Registered Nurse or Clinical Manager at Aglow Recovery Healthcare Services (ARHS) ensures that patient care is coordinated and managed appropriately. The Clinical Supervisor is responsible for ensuring that care and services are delivered according to the individual needs and provides supervision of clinical personnel.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

1. The RN receives case referrals, reviews available patient information related to the case, the disciplines required to determine MRSH care needs, and assigns the appropriate clinicians to the client.

2. Reviews and evaluates the services provided by clinicians, discusses, and verifies clinical interventions, instructs, and guides clinicians to promote more effective client care.

3. The RN conducts Clinical Diagnostic assessments under the supervision of the Clinical Director: this includes but is not limited to client’s provisional diagnosis, current medications, mental and physical health needs.

4. Develops a care plan, which establishes goals based on clinical diagnosis and incorporates therapeutic, preventive, and rehabilitative clinical intervention. Includes the client, ARHS care team, and the client's family in the planning process.

5. Conducts LOCUS and DLA-20 assessments and determines the client's level of care needs, which is incorporated into the individual treatment plans. Performs medically necessary community support care coordination: Regularly re-evaluates patient nursing needs and makes necessary revisions as patient status and change.

6. Attends case conference meetings with organization personnel to facilitate coordination of care. Participate in monthly organizational meetings and treatment planning review meetings as required.

7. The RN provides supervision of the Community Support Workers and Registered Nurses. Reviews notes and evaluates interventions to ensure that they are in congruence with the individual treatment plans.

8. Conducts quarterly record reviews and communicates findings and recommendations to Clinical Director and appropriate organization personnel.

9. Assists in the screening and interviewing process of new organization personnel, makes recommendations for the employment of qualified individuals, and assists in orientation of new organization personnel.

10. Performs other duties and activities as delegated by the Clinical Director. POSITION QUALIFICATIONS

Registered Nurse with current licensure to practice professional nursing in the state.

Two years of management or supervisory experience in health care setting,

Complies with accepted professional standards and practice.

Complies and maintains current CPR certification.

Demonstrates excellent observation, verbal and written communication skills.
