+ Assesses, plans, develops, coordinates, and provides
treatment,rehabilitation, and support services to program
participants.
+ Provides service coordination for assigned Participants including
coordinating and monitoring activities of the individual treatment team
(ITT).
+ Assumes primary responsibility for developing, writing, implementing,
evaluating, and revising overall treatment goals and plans in
collaboration with the Participant and the ITT, providing individual
supportive therapy and symptom management, ensuring immediate changes
are made in treatment plans as Participants' needs change, educating and
supporting Participants' families and advocating for Participants'
rights andpreferences.Minimum Qualifications:
+ M.S., M.A., MSW, or Ph.D. degree in human servicesor health-related
field.
+ Will possess one of the following New Mexico licenses: Licensed Master
Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), Licensed
Clinical Mental Health Counselor (LPCC), Licensed Professional Counselor
(LPC) or Licensed Mental Health Counselor (LMHC).
+ Any candidates who can meet the state requirements to provide clinical
services within their field will be considered.
+ Must possess a valid driver's license and have access to a private
vehiclefor day-to-day job performance.Benefits:
