Responsibilities The Case Manager actively coordinates the care of patients through a multi-disciplinary approach that focuses on the continuity of care from pre-hospital admission to post-hospital discharge. The Case Manager, completes an initial assessment of patient’s physical, cognitive, living arrangements, supportive system status and initial discharge plan. The Case Manager coordinates discharge planning activities (i.e. Homecare Services, DME, Rehab, SNF, OP Services) with active involvement of patient, patient representative, Medical Staff, Nursing Staff, and other disciplines to ensure that the most appropriate discharge plan is implemented for patient’s continuum of care. The Case Manager identifies patient’s psychosocial & socioeconomic issues that may interfere with the patient’s continuum of care progression as well as provides supportive counseling & community referrals (i.e. counseling services, DSS, APS, and CPS) as necessary. Case Managers are also responsible to ensure appropriateness of level care/ patient status and assist in expediting care/treatment while closely monitoring length of stay in a coordinated effort with the Medical Staff, Nursing Staff, and other disciplines as required. Garnet Health System provides a compensation range to comply with the New York State law on Salary Transparency in Job Advertisements. The range for the role is $44.2862 - $54.0620 hourly. The range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits. When determining a team member’s compensation and/or rate, several factors may be considered as applicable (e.g., location, specialty, service line, years of relevant experience, education, credentials, negotiated contracts, budget and internal equity). Qualifications Minimum Education: RN with current NYS License or MSW/CSW with current NYS License CCM preferred/or eligible to sit for CCM exam 3 years from date of hire or prorated for part-time/per diem. PRI and/or Screen certification preferred or willing to obtain certification within 6 months of employment. Minimum Experience: Must be able to communicate and collaborate/cooperate with other team members, payors and community agencies while maintaining and promoting positive relationships, using diplomacy, tact, assertion and compassion. Highly developed verbal and written communication skills. Ability to communicate with a wide range of individuals and groups, and work independently. Must have good organizational skills, judgment, safety awareness, problem solving, and time management skills. Requires knowledge of community resources, third party payors, and reviewing agencies. Prior healthcare experience preferred Schedule: Hours: 8:00am-4:00pm/8:30am-4:30pm/9:00am-5:00pm. Weekend, holiday & on call responsibility on a rotational basis. Physical Requirements: Minimal physical demands-walking, standing, sitting, daily data entry. Able to manage multiple priorities and fast paced environment. Working Conditions: Environmental Demands and Exposure to Hazards: Works in a clean, well-lighted, heated or ventilated facility. No routine exposure to hazards. Physical Demands: Demonstrates physical and functional ability to perform full anatomical range of motion to accomplish tasks. Evidence of visual and aural acuity and finger and hand dexterity to operate computer and office equipment. Can withstand long periods of sitting, standing and/or constant walking. Ability to lift 10 lbs. Mental Demands: Ability to foster collaborative relationships, to work well under pressure, to organize and synthesize new information, and prioritize tasks. Possesses critical thinking, analytical skills and flexibility. Ability to multi-task. Requires detailed attention to work in an environment where interruptions cannot be controlled. Demonstrates sensitivity to customer needs and expectations.
