Rehabilitation Consult Liaison

GENERAL PURPOSE:A rehab consult liaison spends his or her days performing a variety of actions, all of which are directly - or indirectly - related to that end goal of bringing patients into the facility. But it¿s not just patients in beds that make a liaison successful. Those patients need to be appropriate for acute inpatient rehab in the first place. A rehab liaison will spend his or her days in a variety of ways, including, but not limited to: responding to evaluation orders, evaluating patients (not physically but in more of a case manager role), admitting as many appropriate patients to their facility as possible, obtaining as many patient referrals as possible, spreading the word about the facility to physicians, nurses, case managers, and the general public and representing the facility at various marketing and social functions.RESPONSIBILITIES:

Ensure patients are identified and prioritized accordingly for successful submission and insurance authorization for admission to the inpatient rehab units.Maintaining up to date consult status and lists to ensure continuous pool of patients to be admitted to 512 and 61200 are available from both within SMH as well as affiliates and other sources of viable patients to maintain maximum capacity of units while maintaining compliance with CMS regulations.

Develop relationship with patients from time of rehab consult through discharge from Acute Inpatient Rehabilitation. Establish from time of consult and maintain collaborative relationships with families/caregivers, multidisciplinary provider(s), PT, OT, SLP and TR, nursing and other internal service delivery departments.

Assures continuity of care:communicates pertinent information regarding patient issues with other team members; recommends services consistent with the patient¿s care needs and benefits; coordinates patient education and care; serves as ongoing resource for patients and families from time of rehab consult through discharge from acute rehab; collaborates with other care providers regarding how to best meet the needs and sequence care; assesses patient¿s learning style and health literacy to provide education at the appropriate patient level.

Participates in development of standards, the implementation and evaluation of policies and procedures; the development of compliance with treatment guidelines and in the assessment of practice patterns, identifying needed changes and establishing measurable action plans.In partnership with data management staff, collects and report data quality measures consistent with organizational needs.

QUALIFICATIONS:Required:

Bachelor of Science in a clinical field

3 years Relevant experience with a minimum 2 years in a rehabilitation hospital setting

Rehabilitation Therapy, Nursing, or Respiratory Therapy experience

or equivalent combination of education and experience

Strong interpersonal skills, outstanding clinical judgment, and critical thinking

Must be self-directed, good task manager, and have skills in electronic device navigation

Ability to navigate an electronic health record efficiently is necessary

Licensed Registered Nurse Upon Hire or

Registered Nurse Multi State License Upon Hire or

Licensed Physical Therapist Upon Hire or

Licensed Occupational Therapist Upon Hire or

Licensed Speech-Language Pathologist Upon Hire or

Registered Respiratory Therapist - NBRC Upon Hire
