Social Work Assistant (BSW)

GENERAL PURPOSE:The social work assistant (BSW) is responsible for the implementation of phone triage and psychosocial assistance for patients aged 18 yrs. and older and for ensuring continuity of care upon discharge for the Adult Ambulatory Services. They participate in the planning and delivery of appropriate service linkages, and serves as liaison and advocate for patients and their families with identified service providers. The social work assistant (BSW) is a member of the interdisciplinary treatment team for the patients.RESPONSIBILITIES:

Conducts telephone screening on new referrals to ensure appropriateness for clinic services. Subsequent to phone screening, assigns patient to most appropriate intake slot. May administer screening tools and conduct record review.

Acts as a backup to clinic access coordinator as a liaison for acute services to schedule and coordinate priority access appointments. Also provides back up services to the BSW for scheduling new intakes to Adult Ambulatory Services via the work queue, phone call, or fax.

May assist the clinical team with patient, family and agency follow up via outreach, scheduling and collaboration with current providers and social supports.Assist to both collect data and to assess agency involvement.

May assist the clinical team in assessing needs of patients and completing referrals, such as but not limited to, those involving transportation, care management, etc.

Monitors requests for patient records.

Attends Division, Department, and Clinic level staff meetings. Ensures that Division, Department, and Clinic policies and procedures are implemented with regard to professional practice and patient care. Ensures timely completion of statistical reports/forms, time sheets, and other Hospital or Division mandatory in-service trainings and Health Assessments. Subscribes to maintaining a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences. Participates in available cultural competency events and training appropriate to job duties.

May provide case management services to active clinic patients.

May provide social work follow up to patients, families, and agencies as needed after discharge to ensure continuity of care and integrity of the discharge plan.

QUALIFICATIONS:

Bachelor¿s Degree in Social Work from an accredited program required.

Experience working with an interdisciplinary team preferred.

or equivalent combination of education and experience

Strong organizational skills preferred.

Ability to communicate effectively with a diverse group of medical providers preferred.

Ability to communicate effectively in written form preferred.
