GENERAL PURPOSE:The Social Worker is responsible for providing clinical services to families of children who admitted to the Neonatal Intensive Care Units and to other areas of the Hospital when the NICU is over census. The Social Worker is also responsible for psychosocial intervention and discharge planning for select inpatient mothers of NICU patients.RESPONSIBILITIES:
Provides social work services including psychosocial risk assessments to antenatal and postpartum mothers and their significant others whose babies are in the NICU. This includes addressing psychosocial issues around having a sick or congenitally malformed infant, adjustment to parenting, maternal substance abuse, maternal depression and other behavioral health disorders, domestic violence, parental discord, financial concerns and linkage to other services and resources. It also includes short-term bereavement counseling to families who have experienced a neonatal death.
Attends interdisciplinary rounds and collaborates with members of the NICU healthcare team around matters related to patient care and discharge planning.
Identifies risk management issues, including cases of child abuse and neglect and communicates them to the assigned Social Work supervisor and to the Social Work Associate Director.
Documents social work activity in the NICU patient¿s medical record and in the mother¿s hospital record
Collaborates with other social workers to ensure continuity of social work care between clinical settings.
Provides concrete services to families needing assistance with housing, transportation and meals during the hospitalization.
Provides counseling for patient and families around special Medicaid programs for supplemental medical insurance and medical care at home.
Serves as liaison to health and social service agencies involved in the care and support of patient families.
Other duties as assigned
Participates in activities of the Social Work Division including, but not limited to staff meetings, time reporting and other Hospital or Division forms; Hospital and Division mandatory in-service training programs, and UHS reviews. Participates in ongoing professional development
May participate in teaching of medical, nursing and social work staff.
May serve as a field instructor of social work students.
May provide consultation and teaching to Hospital staff on psychosocial issues in the health care of high- risk infants and families.
QUALIFICATIONS:
The Social Worker must hold a Master of Social Work degree from an accredited institution. New York State licensure (LMSW) is required within eighteen months of employment.
Experience in a medical setting and/or maternal/child program is preferred.
Knowledge in psychosocial assessment, safe discharge planning, comfort with complex medical conditions preferred.
