Description:Patient Care Coordinator - Pharmacy Technician (intake) Key Responsibilities:
review contents of referral to ensure complete and accurate info received
create patient profile from a scan or fax image by entering correct patient and provider info
conduct initial check of prescription to ensure clinical accuracy for processing
communicate with provider regarding referral status, prescription clarification, missing info, or patient concerns
updating concise progress notes to ensure other roles within the workflow can quickly assess the status of referral
enter in new provider information
Qualifications / Requirements:
High school graduate or equivalent
2 years of experience in Pharmacy
Pharmacy Technician License
