You will participate in discharge planning rounds to determine the discharge needs of the NICU patient.
You will coordinate family meetings to include scheduling, preparing, attending and follow up.
You will act as the liaison between the physician, family and staff for coordination of care and discharge planning.
You will act as a resource for the bedside nurse to coordinate discharge planning activities.
You will participate in establishing, maintaining, and improving patient’s health care goals through patient assessment, planning and implementation of nursing interventions and evaluation of patient outcomes.
What qualifications you will need:
BSN upon hire or ADN with completion of BSN within 24 months of hire
Current RN license in Texas, BCLS certified, Medication Learning Module, NRP
Certification in specialty area preferred
Unit specific clinical experience required
