Job SummaryThe Case Manager serves as a team member with several social service professional and paraprofessional staff ensuring the delivery of quality care in accordance to the rules and regulations established by the Los Angeles Department of Mental Health for contracted agencies as well as adhering to the mission of St. Joseph Center.ESSENTIAL DUTIES & RESPONSIBILITIESThe Case Manager’s key areas of responsibility include:  Key Areas of Responsibility: Provide targeted outreach, engagement, and education activities to Latino individuals and families in order to increase awareness regarding overall mental health and health issues.Perform routine case management activities while working with other program staff providing services to clients.Essential Duties: Help prospective clients gain access to needed community resources and support.Assist in establishing for services as defined by agency and funding sources.In conjunction with other staff, coordinate activities, recruit participants, arrange event logistics, and provide staff support.Facilitate therapeutic groups, including working alongside with other staff to develop and implement curriculumsParticipate in regular Latino ISM team meetings.Document all advocacy and case management activities as required by agency and/or funders.  Other duties and responsibilities may be assigned.
