Adult health care in Santa Clara has a part-time or contract LCSW or Psychologist or a Psych nurse for the following position:
POSITION DESCRIPTION: As part of the Multi-Disciplinary team and in coordination with center’s MSW, conducts assessments and creates care plans for the elderly participants.
DUTIES:
1. Participates in the initial and continuing assessments for the development of an individual plan of care by the Multidisciplinary Team.
2. Maintains signed progress records in the participant's health record and documents all treatments as they are provided to participant.
3. Provide one-on-one consultations to participants
CONDITIONS OF EMPLOYMENT:
Must have current CPR and First Aid certifications. Must have had a TB clearance within the last 12 months or within 7 days of employment.
