ResponsibilitiesGENERAL DESCRIPTION:The social work assistant is responsible for the implementation of psychosocial assistance and for ensuring continuity of care for Strong Ties patients. They participate in the planning and delivery of appropriate service linkages, and serve as a liaison and advocate for patients and their families with identified service providers. The Social Work Assistant is also a member of the interdisciplinary treatment team for patients at Strong Ties Clinic.RESPONSIBILITIES:
Assists the clinical team with patient, family and agency follow up via outreach, scheduling and collaboration with current providers and social supports.
Assists the clinic access coordinator in being a liaison for acute services to schedule and coordinate priority access appointments.
Assists group therapy service coordinator with scheduling group screens.
Assists the clinical team in assessing needs of patients and completing referrals, such as but not limited to, those involving transportation, care management, etc
Attends Division, Department, and Clinic level staff meetings. Ensures that Division, Department, and Clinic policies and procedures are implemented with regard to professional practice and patient care. Ensures timely completion of statistical reports/forms, time sheets, and other Hospital or Division mandatory in-service trainings and Health Assessments. Subscribes to maintaining a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences. Participates in available cultural competency events and training appropriate to job duties.
May provide social work follow up to patients, families, and agencies as needed after discharge to ensure continuity of care and integrity of the discharge plan
QUALIFICATIONS:
Bachelor’s Degree in Social Work from an accredited program required.
Experience working with an interdisciplinary team preferred.
or equivalent combination of education and experience required.
Strong organizational skills preferred.
Ability to communicate effectively with a diverse group of medical providers preferred.
Ability to communicate effectively in written form preferred.
