Job Description Provides quality, cost-effective inpatient/outpatient and/or community care for clients and families in a target population across the continuum of care. Works in coordination with interdisciplinary teams to organize, prepare and deliver evidence based clinical services reflected by a multi-disciplinary plan of care. Job Responsibilities 1. Evaluates the psychiatric and psychosocial needs of the patient/client to assess and plan care. Establishes a multidisciplinary plan to address identified issues and goals. 2. Utilizes individual, couple, group, and family therapy specific to address each patient’s/client’s needs as part of the multidisciplinary treatment team in accordance with the treatment plan. Organizes and prioritizes care, interventions, safety planning, and treatment. Identifies and stewards appropriate resources in order to meet patient/client and family goals. 3. Provides client medical documentation based on program needs including but not limited to social history, treatment plans, progress notes, discharge, assessment, and After Care Plan in accordance with regulatory licensing requirements and third party agencies. May diagnose patients/clients using indicated DSM criteria. 4. Coordinates and implements client/patient/family care along the continuum by collaborating with the multidisciplinary team, payers, community and regulatory agencies to provide an integrated delivery of healthcare. 5. Educates patient/client and family members and ensures seamless, coordinated transitions across levels of care to meet established goals. Refers patient/client and families to appropriate levels of care, including support groups and to other available resources in order to facilitate recovery and/or treatment process. 6. Provides direct patient/client care including individual, couples, family therapy, group therapy, advocacy, referral and discharge planning. Performs case management and case coordination: arranges aftercare and other necessary services. 7. Regularly evaluates goal directed effectiveness and efficiency of plan, documents patient/family responses, identifies variances from outcomes and revises plan of care to meet patient/family needs. 8. Negotiates level of care and length of stay with managed care companies and informs the third party payers of ongoing changes in the client’s status. 9. Works across programs for clients with both mental health and substance use disorders. Demonstrates competencies, including knowledge and behaviors, to meet age specific patient needs. Ensures personal, patient/client and unit/staff safety. Adheres to entity policies and procedures, reports and responds to risk management issues in a timely manner to safeguard the entity from adverse events. 10. Participates in administrative and development activities including team meetings, clinical supervision of staff and staff education. Supports the development of hospital and/or unit based programming to meet the clinical and strategic needs of the clients/patients and the milieu. Works effectively as a team member to contribute to improved patient/client experience. Ensures the cost-effective utilization of resources. 11. Perform other related duties as required which may include but are not limited to:Providing supervision and/or clinical supervision for interns and other staff as assigned. Providing on-call coverage. Master’s Degree in Social Work, Counseling, or Related Field Three years’ experience providing direct services to psychiatric patients, substance use population. Previous experience in assessment and diagnosis and individual/group therapy and crisis intervention. Active CT License in Social Work, Professional Counseling, Alcohol & Drug Counselor, Marital & Family Therapy. Conducts Emergency Evaluations for clients in the community if required by the program. 10. Processes referrals and requests for service from prospective clients and professionals, directs requests to the most appropriate level of care. Responsible for bed management for Rushford residential programs with a focus on excellent internal and external customer service. Maintains a strong working knowledge of all applicable computer systems that affect and support bed placement and transfer activities. Job Responsibilities 1. Triages clinical needs of callers/referrals. Utilizes scripted Rushford screening tool and decision tree to direct prospective clients to the most appropriate residential or ambulatory level of care for assessment services. 2. Listens for urgency & crisis to coordinate and/or involve emergency services as necessary. 3. Partners with physicians and clinical leadership in the communication of clinical issues related to client placement/flow. 4. Schedules ambulatory and residential assessments, adhering to Rushford standards for emergent, urgent, and routine needs. 5. Focuses on efficient bed management to ensure open beds are filled as soon as possible. 6. Refers clients to appropriate alternate facilities when Rushford is unable to meet these needs. 7. Processes direct referrals from professionals with attention to medical acuity, psychiatric stability, and prescribed medications. 8. Accurately collects and enters demographic and insurance information from prospective client and/or referral sources into business software. 9. Verifies eligibility and benefits on the Department of Social Services Website and Availity or other web sites for commercial providers. 10. Maintains point of clinical contact with outside referral sources to secure clinical information regarding pending transfer activity and resultant client placement needs. 11. Achieves seamless delivery of services by appropriately involving colleagues, physicians, nurses and other staff to ensure commitment, communication and cross-functional linkage. 12. Collaborates with Admission and Program Managers to achieve utilization goals and ensure availability and accuracy of intake appointments and access to services. 13. Effectively manages bed resources to support client demand. 14. Ensures the smooth functioning of the bed management process in order to reduce wait times and avoid unnecessary movement of clients. 15. Serves as point of contact to communicate with physicians and nurses regarding availability of direct admissions and transfers to support timely placement. 16. Supports unit/department management in the facilitation of inter- and intra- unit problem resolution. 17. Partners with clinical leadership and management to define, reduce, and eliminate processes that may negatively affect client throughput/efficient delivery of care and implement improvements. “Study and adjust” each change to achieve cost effectiveness and desired result. 18. Increase effective bed capacity by identifying barriers in bed assignment processes and collaborate with clinical management in the resolution of these issues. 19. Documents and reports on timeliness of client placement activities and issues of concern related to efficient throughput. 20. Pulls client forward as unexpected beds become available. 21. Monitors clinical and fiscal goals and objectives to address specific issues and support throughput efforts. 22. Develops effective relationships with Transportation options to enhance client flow and bed management. 23.
