Summary The CNH RN Coordinator is responsible for coordinating a variety of treatment services and administrative needs including outreach, care management services, discharge planning, multidisciplinary activities, assessments, and the collection and maintenance of facility inspections, reviews of state reports, nursing homes compare sites data, updating National-driven data for compliance and monitoring of needs for the fiscal obligations and contract services within CNH facilities in the WCHCS area. Responsibilities Makes regular, frequent and thorough security rounds as prescribed by policy. Is available to assist visitors and staff, as well as ambulance, police, fire and helicopter personnel. Intervenes with difficult or disruptive patients and/or visitors with the lowest level intervention possible. Problem-solves situations and acts in a well-thought out manner. Reviews pass on information with oncoming shift, any posted notices and the previous shifts\' logs before the beginning of each shift. Reports any problems or needed maintenance to Engineering.
