Helping Hands is looking for a Full Time Registered Nurse to work in our Hospice Division. Job Description As a Hospice Registered Nurse (RN) you will be helping patients and their families navigate a very difficult time in their lives. Utilizing hospice best-practice care delivery approaches, you will have all the tools and resources needed to ensure a patient-centered end-of-life experience. As a member of an interdisciplinary team, you will be able to work with experienced teammates who care deeply about delivering a unique experience for every patient. What you will be doing: Symptom management: Through a thorough assessment you will work closely with the patient’s attending physician and the hospice Medical Director to ensure every patient’s symptoms are well managed. Plan of Care: Evaluate the treatment and medical condition of the patient according to the plan of care. Individualize the plan of care and adjust as needs change. Continuity of Care: Manage interdisciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges. Family Educator/Advocate: Counsel patient and family on the disease/progression and incorporate patient and family in the development of the plan of care. Listen to patients and family members. Documentation: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines. Operations: Oversee and supervise total care of patients provided by nurse aides and LPNs. Attend in-service training and mandatory agency meetings. Skills needed: Exceptional communication and a positive attitude. Patience is a virtue when working with patients, families, physicians, and coworkers. Attention to detail is critical, as is being observant and following directions. Education: Graduate of an accredited registered nursing program. Bachelor’s degree in Nursing preferred. Certified Hospice and Palliative Nurse (CHPN) certification preferred Experience: Minimum of 2 to 4 years as a RN with experience in a hospice or home health agency or other related health program. Experience with pain and symptom management interventions preferred. Ability to work within an interdisciplinary setting. Licensure, Registration, and/or Certification: Must have a current Registered Nurse license in good standing in the State of Pennsylvania in accordance with the Pennsylvania Board of Nursing in accordance with the Pennsylvania Revised Code 4723. Valid driver’s license and reliable transportation that is insured in accordance with Pennsylvania state requirements. Satisfactory references from employers and/or professional peers. Satisfactory criminal background and national sex offender checks.
