REGIONAL MANAGER
Responsible for coordinating, monitoring client status, needs, changes, reassessments and complaints. Coordinate a wide array of healthcare services for individuals, including scheduling appointments with specialists and coordinating home care support. Assist in coordinating and scheduling caregivers for patients, ensuring that all shifts are appropriately filled. Conduct assessments and gather necessary information to determine care plans for individuals with a variety of physical disabilities or chronic illnesses and program eligibility. Act as a liaison between patients, families, and healthcare providers. Complete new client intakes and explain rules and regulations to clients. Ensure seamless delivery of services, manage multiple tasks, communicate effectively with staff and caregivers, and utilize various app-based tools to facilitate smooth operations. Oversee the recruitment, hiring, and training of home health care staff, ensuring they meet all regulatory and company standards. Manage patient records and ensure accuracy of information. Document and report to supervisor when there is a change in client status, change in service requirement, change in aide assigned to case, incidents, and complaints.
