Responsibilities The Case Manager actively coordinates the care of patients through a multi-disciplinary approach that focuses on the continuity of care from pre-hospital admission to post-hospital discharge. The Case Manager, completes an initial assessment of patient’s physical, cognitive, living arrangements, supportive system status and initial discharge plan. The Case Manager coordinates discharge planning activities (i.e. Homecare Services, DME, Rehab, SNF, OP Services) with active involvement of patient, patient representative, Medical Staff, Nursing Staff, and other disciplines to ensure that the most appropriate discharge plan is implemented for patient’s continuum of care. The Case Manager identifies patient’s psychosocial & socioeconomic issues that may interfere with the patient’s continuum of care progression as well as provides supportive counseling & community referrals (i.e. counseling services, DSS, APS, and CPS) as necessary. Case Managers are also responsible to ensure appropriateness of level care/ patient status and assist in expediting care/treatment while closely monitoring length of stay in a coordinated effort with the Medical Staff, Nursing Staff, and other disciplines as required.
